  • Brief Introduction
  • Briefly identify the clinical issue – including how it was discovered – Charts, staff, observation, evaluations
  • Briefly identify the root cause(s) – including how you found the root cause(s)
  • A brief summary of aggregate data – describe how you got the data – from risk management, benchmarks, national databases, financial
  • List or table of major barriers and facilitators within unit/organization
  • A brief list of recommendations – potential actions to resolve the issue
  • Brief conclusion
