Summative Assessment Brief

Within this module you must complete a 3,000-word written submission. There are two parts to the summative assessment, and you must complete both parts within this total word count. We have outline below an estimate of how you may want assign word count.

1. From the range of tools provided, select a tool used to support anticipatory care planning and provide a critical analysis of the efficacy of this within the context of your field of practice (1500 words)

You should consider

• How this meets with the clinical aims
• How this meets national/local policy aims and objectives
• What are the strengths and weakness of this tool
• If there are other more relevant tools

2. Provide a critical reflection of the leadership and change management skills that would be required to implement this within your most recent clinical practice experience (1500 words)

You should consider

• What is your leadership style and how might this affect implementation
• What might be the facilitators to change
• What might be the barriers to change

You must demonstrate engagement with the evidence base throughout all elements of the assessment, including your critical reflection. You should ensure that you use a recognised model of reflection to structure your discussion.

All in text and end of text references must adhere to the School of Health and Social Care style − APA 7^th. The reference list presented at the end of your writing should be in alphabetical order by the first author surname. References should be written in full, no bullet points or numbers assigned to individual references in the reference list. Use the APA 7th quick reference guide to help you.
