survey in Qualtrics.
- Understand how to use skip logic and branching to tailor the survey questions based on
participants’ responses.
- Understand how to generate a survey link in the chosen platform.
- Understand how to customize the survey link to include relevant information such as the
survey title or the name of the needs assessment case study.
- Understand how to distribute the survey link through various channels, such as email,
social media, or web pages such as Blackboard.

The Assignment
Survey construction refers to the process of designing and creating a questionnaire or survey instrument to gather information or data from a specific population or sample. The goal of survey construction is to create a valid and reliable tool that effectively measures the constructs of interest and generates meaningful data that can be analyzed and interpreted.
When constructing a survey with demographic questions and a scale or measure related to healthcare outcomes, healthcare policy and management students can benefit in several ways.
Firstly, demographic questions can help identify the characteristics of the population being studied, which can help inform healthcare policies and management decisions. For example, knowing the age, gender, income level, and educational background of patients can help healthcare professionals tailor services and programs to better meet their needs.
Secondly, including a scale or measure related to healthcare outcomes can help assess the effectiveness of healthcare policies and interventions. For instance, if a survey includes questions about patients’ health status, satisfaction with care, and quality of life, researchers can use this information to evaluate the impact of various healthcare policies and programs.
Overall, constructing a survey with demographic questions and a scale or measure related to healthcare outcomes can provide valuable insights into the needs and experiences of patients, which can inform healthcare policy and management decisions and improve the quality of care provided.
Here are the steps for creating a survey with demographic questions and a scale in Qualtrics:
- Log in to your Qualtrics account and select “Create Project” to create a new survey.
- Choose a survey template or start with a blank survey.
- Add your demographic questions by selecting “Add Question” and then selecting
“Demographics” from the question types. You can include questions such as age, gender,
education, income, and occupation.
- Add your scale questions by selecting “Add Question” and then selecting “Multiple
Choice” from the question types. Then, select “Matrix of Dropdown Menus” as the question format. You can include a range of scale questions, such as Likert scales, semantic differential scales, or numerical scales.
- Customize your questions by adding answer choices and labels. You can use the pre-built answer choices or create your own.

- Add any additional questions or survey elements that you want to include.
- Preview and test your survey to make sure it functions correctly.
- Distribute your survey by sharing the survey link or posting it on Blackboard as a Word
document.
These steps should give you a basic overview of how to create a survey with demographic questions and a scale/measure in Qualtrics. However, there are many additional customization options available in Qualtrics, so feel free to explore and experiment with different features to create a survey that meets your specific project needs.
As an additional resource, the following step-by-step instructions are provided by Qualtrics. It is recommended that you click on the links below and follow the steps.
