Summary: This is where you will upload your second case study from the IHI Introduction to the Triple Aim module.Please make sure that your case study includes the following gradable components:your name at the top of the case studyan introductory summary of the casea list of the questions asked in the case and your answers (list your question and then the answer)APA references as appropriateWhile you don’t need a cover page, you do need your full name on the top of the paper so I know it is yours. Yu need an introductory case summary so we know what the case is about and the paper should be a minimum of one academic paragraph (minimum of 5 sentences) per question asked in the case study. You must answer all questions in your chosen case study to receive full credit for the practicum hours. If references are used, they must be cited appropriately. This is a pass/fail assignment and failure to complete this deliverable will result in a practicum course failure. Late penalties apply per the syllabus and no work may be accepted after 5 days late.
