Patient Identifier: Includes patient’s initials, age, and ethnicity.

 Current Problem List

Includes updated problem list. Patient must have at least 1 active diagnosis.

Medication List, current medication list (drug name, dose, frequency).

Includes past medical history including previous diagnoses, surgeries, procedures, and hospitalizations.

Includes living situation, employment, drug/alcohol/tobacco use, social support, and sexual history.

Family History

Includes family medical history organized by either family member or disease (family members include grandparents, parents, siblings, children, and pertinent 2nd degree relatives). You would include if they are alive and well, if they have any diseases or conditions, and cause of death for those you have passed away.

Health Promotion; Includes health screenings (breast, cervix, colon, lung, etc.), immunizations, nutrition, and physical activity level.

Health Risk; Include any environmental exposures or any other health risks.

Health Priorities

Include top health priorities for this patient (for example, one might be that patient needs to get mammogram since she is overdue and has a family history). Must include a minimum of 3 health priorities). Include evidence-based resources to support your answers (i.e. why it is a priority, current guidelines, etc.).
